What happens next?
- You will be given a copy of the form.
- The worker will discuss your information and ideas with a team.
- They will choose someone to be your Lead Professional.
- There will be an Action Plan where they write down what people can offer.
- Everyone who has offered to help will do what they have agreed.
- Your Lead Professional will look again at the Action Plan with you to see how things are going.

What is a lead professional?
They can be a worker from any service. It may be someone you already work with or someone new. They will keep you informed, listen to your views and support you.

Confidentiality
Your information will be kept securely and shared with those people who are likely to be working with you and your family.
A young person's wish to keep information confidential from their parents will be respected when this is in their best interests.
There may be certain times when the people working with you need to share information with more people. For example:
- When they need to find out urgently if a child or adult is at risk of harm.
- To help prevent or detect a serious crime.
